We’ll send you intake forms

Our intake forms ask for standard information such as your health history and current needs. This helps your provider prepare and focus on what matters most to you.

We’ll review your insurance

If you’re using insurance, we’ll review your coverage and confirm whether we’re in network. We’ll do our best to explain your benefits clearly, and we’re happy to help with any questions.

We’ll send your visit link

For virtual visits, we’ll send you a secure, confidential link to connect with your provider at the time of your appointment. All information you share with us is private and secure.
